The guy on the stretcher looks like an actor who’s signaling to the prompter for his next line.
I thought much the same, was going to say that he definitely looks like someone having a medical crisis and not at all an actor in a CPR training video.

This cover made me laugh a little so I hope that the book is a comedy.
