I think that is supposed to be seen as an inner tube next to a swimming pool. But it certainly looks like hospital equipment.

It’s admittedly a poor choice to put that image on the cover, but I don’t necessarily think it’s fatal. No one is likely to die over it. Heck, even the author’s career might live through the experience if she has the sense to join the “New Covers for Old” club!
