Among my pet peeves are covers that may be passable full size but are illegible and indistinct at thumbnail (think the presentation bar of “suggested titles based on your browsing history”).
This pixelated cover actually looks better at thumbnail. But even for e-books, covers ought to be distinct and readable at any size used in sales and advertising.

This is one of those covers that tricks me into thinking the lo-res preview has downloaded and I wait and wait for the hi-res jpeg to download, cursing my slow internet connection, until I realize that it is a low res cover, then I have to apologize to my modem.
