The back of the title page (the credits) includes this disclaimer:
“The cover design and editorial work for this book are entirely the product of the author.
“Gatekeeper Press did not participate in and is not responsible for any aspect of these elements.”
We actually have a remotely similar, but more polite, disclaimer that we use on the copyright page (or wherever):
“Interior layout and design created by [Jane Doe] with technical assistance provided by [Us].”
I got that from a fellow bookmaker and it’s served me well. I just had a customer insist on some things for her book that are KILLING me and I quietly told my guys to add that. With some customers, I live in dread that they tell all their buddies at the XYZ Writing Group that we did their layout. 🙂
