Yup, that’s pretty much my reaction and I was serving, in that era, too. (In Germany, not Nam, but my peers were serving there and) Even I have no goddamned Idea what he’s talking about with that title.

All I can find military on “head first” relates to funerals. Usually the casket is carried feet first to the burial site; a chaplain’s casket is carried head first.

“Head First” is Book 2 of 3 in the “Cooper Vietnam Era Quartet”.There may be a fourth book forthcoming but this book is listed at Amazon as “Book 2 of 3: Cooper Quartet”
The cut and paste cover shows a helicopter in the air in the jungle (stock?) and news clip of a kid fleeing a carpet bombing pasted colorized running toward the ‘copter.

To be fair, I believe that’s entirely Amazon that adds that little bit in, and they can only count books that the author has already created.
