Either hire an artist or change the name to The Cancer Survivors.
Another example of someone who needs to spend more time learning the craft and less pretending to already be an expert while doling out bad advice online and selling “How-to write” books. Is it a handbook or a tool belt? Pick one for cripe’s sake.
You’re right. I’d be willing to bet this cover was meant as a mockup to be redrawn later. I’ve had several authors take a mockup I created for them and use it as their cover even though I was planning to create full covers with proper compositing and color grading.

Oddly, looks like he did hire an artist: the back cover of the print edition credits Jack Baker, with this website: https://www.jackbakerart.com/
The work there seems stylistically consistent but noticeably better though.
