I thought surely the balls and the huge willy must be intended to be part of the plinth the man is sitting on, but really I can’t see how they could be intended to be anything else. I tried reading the book’s description, but it doesn’t seem to give any sort of clue what the book is actually about.

Yup, it’s a guy fondling his big, um, hammer. But I too find the balls interpretation a lot more interesting. Even though the handle of the hammer is a bit, um, thin for my taste.

Hopefully we’ll see a full-blown Pygmalion style animation of the drawing. And that hammer will finally see some action, directed at “Box of Garden Soil.”
